Saurimo, Angola: whilst transferring diesel from a road
tanker trailer into a Samil fuel truck, the fuel was transferred
in a manner so that the tanker counter balanced to the front of
the truck causing the trailer to tip onto the jack legs. There
was ~10,000 litres onboard at the time and very lucky to not
lose the whole load in the event that the jack legs collapsed.
Key learning's:
Although this was a relatively simple task, the risk of the
trailer counter balancing was overlooked. The organisation never
learnt from past mistakes identical to this situation to ensure
that these situations were never repeated.
