"LPG Lift Truck Fire: Lift truck was serviced by vendor the
day before. No pre-shift inspection was completed and truck was
put into service on the following day. Vendor technician left
grease rag on top of truck manifold. Engine heated up and rag
caught fire. Operator properly dismounted, retrieved fire
extinguisher and put fire out before further damage or injury
could be sustained"
