This was an adaptor cable used to
convert a 16Amp BS4343 connector to domestic UK Mains,
to enable a caravan to be powered up when not on a
touring site.
The live pin had over heated and
melted the plug case. It was found that the ferrule
fitted to the live conductor was loose, so even though
the terminal screw was tight, the conductor strands
could move in the ferule.
Because the cable
clamp was not tight, the cable flex could move around
and the live conductor could slide in the ferule so that
strands could come out and get bent back, resulting in
fewer and fewer strands being available to carry the
current.
It was found that the ferrule fitted to the live conductor was
