"This man had rendered the lower part of the wall and then walked up to the second level on the scaffold using the diagonal piece of the scaffold to get up.  No ladders were used.  Members of the public walking below the scaffold and bits are falling down where the individual is working."

"Take note of the young man sitting on the grit bin taking a rest whilst his partner is working on the top level.  Earlier, whilst the man was working on the top level, he dropped something which hit this individual below on the head.  No hard hats are worn.  When he runs out of cement, the lad at the bottom, passes up the bucket by standing on tip toes and the individual above bends down to accept the bucket. Note at the top there is no fall protection from this level"
