This photo is of a group of roofers working at a
facility Ohio, USA. They were lifting a bottle of propane to the
roof. Unfortunately, the spot they selected to set up their
hoist was right over an entrance door to the plant. When this
photo was taken, the hoist had jammed and one of the roofers was
working to un-jamb the hoist rope while standing on a stepladder
at roof’s edge, securely anchored by two fellow workers. No fall
protection was used by any of the crew.
At this point, the contributor could no longer watch the
possible fall, but contacted the owners to let them know what
was occurring.
